The Administration of the Faculty of Medical Technology and Rehabilitation in Ubari hereby announces the commencement of student enrollment for the current academic year. Applications are accepted from high school graduates (Scientific Section) for both the first and second rounds, subject to the following terms and regulations:
First: Admission Requirements
- The applicant must be a graduate of the high school class of the current academic year.
- The applicant must hold a cumulative grade of no less than (Very Good).
- The applicant must bear a record of good conduct and exemplary behavior.
Second: Required Documents and Credentials
- Original High School Certificate (Transcript).
- Recent Birth Certificate + Recent Residency Certificate.
- Certificate of Good Conduct.
- Valid Medical Clearance Certificate.
- Copy of National ID Card or Passport.
- Copy of Family Record Book or Family Status Document.
- Six (6) recent passport-sized photographs.
- File folder for document submission.
Third: Registration Timeline
- Start Date: Tuesday, August 25, 2026.
- End Date: Tuesday, September 8, 2026.

The Faculty Administration urges all prospective students to strictly adhere to the designated registration period and to ensure that all required documents are complete and submitted inside the file folder. No applications will be considered after the deadline.
